The postoperative complications of TECA-LBO in dogs, with special emphasis on neurological changes

Abstract:

Between February 2019 and September 2020 we followed up dogs admitted to the Department of Surgery and Ophtalmology of the University of Veterinary Medicine, who arrived for Total Ear Canal Ablation and lateral bulla osteotomy (TECA-LBO), due to end-stage otitis externa complicated by otitis media in most cases. During this period we enrolled 23 patients in our prospective clinical study. The aim of this study was to find correlations between postoperative complications, especially neurological changes and the medical history, events during surgery (bleeding, facial nerve involvement, condition of the tympanic cavity), and the bacteriological culture result from the bulla.

Our hypothesis was that these conditions, through their predisposing effect, may play a role or promote the possibility of postoperative neurological and other complications.

For all 23 patients, a detailed medical history was recorded prior to surgery, and then all major intraoperative events (facial nerve involvement, bulla condition, bleeding rate, bacteriological sampling results) were recorded and classified. Before surgery and the day after surgery, a Schirmer test was performed to measure tear production. After leaving the clinic, telephone interviews were performed (2 weeks, 2 months, end of the study) to further characterize the postoperative complications.

According to the results, the chances of postoperative neurological complications were lower with the facial grade 0 during surgery and in the patients where anaerobic bacteria were detected from the tympanic cavity isolate, a significantly higher proportion of neurological symptoms occurred (p = 0.002051). Other factors had no significant effect on postoperative complications.
